Bring All Insurance Cards

Parent must be present with child under the age of 18

Consent and insurance forms will have to be filled out by all who wish to get the vaccine.

Both Nasal and Injection will be available.

If you wish to fill out the forms ahead of time and bring them with you; they will be available at the Town Clerk's Office, starting on Tuesday, January 5, 2010.
